GSK836 Bepirovirsen: First Hepatitis B Functional Cure Nears FDA Approval

By CII-Contributing Analyst
16.06.2026 9 Min Read

What Happened

GSK’s Bepirovirsen (GSK836/GSK3228836), a first-in-class antisense oligonucleotide (ASO) developed with Ionis Pharmaceuticals, is on the verge of becoming the first drug to achieve a functional cure for chronic hepatitis B. In January 2026, GSK announced that the Phase 3 B-Well 1 and B-Well 2 trials both met their primary endpoints — a landmark moment for a disease affecting approximately 300 million people worldwide and for which no curative therapy has ever existed.

The U.S. Food and Drug Administration granted Bepirovirsen Breakthrough Therapy Designation and Priority Review, setting a PDUFA target action date of October 26, 2026. If approved, Bepirovirsen would become the first therapy to offer a functional cure — defined as sustained HBsAg loss with undetectable HBV DNA — to a meaningful proportion of chronic hepatitis B patients. Current standard-of-care treatments (nucleos(t)ide analogues) merely suppress the virus and require lifelong administration, with an annual functional cure rate of approximately 1 percent.

In a separate but strategically significant development, Sino Biopharmaceutical (HK: 01177) subsidiary Chia Tai Tianqing announced on May 11, 2026, that it had secured exclusive commercialization rights for Bepirovirsen in mainland China, Hong Kong, and Macau. The partnership positions GSK to access China’s enormous hepatitis B patient population — estimated at 70 million carriers, the largest in the world — through the country’s leading liver disease pharmaceutical platform.

The Phase III clinical data were presented at the European Association for the Study of the Liver (EASL) 2026 Congress on May 30, 2026, providing the broader hepatology community with the most comprehensive look at the drug’s efficacy and safety profile to date.

Key Developments

1. Phase 3 B-Well Trials Deliver Landmark Results

The B-Well 1 and B-Well 2 trials enrolled a combined total of over 1,500 patients with chronic hepatitis B across multiple global sites. Both trials evaluated a once-weekly, 300mg subcutaneous injection regimen of Bepirovirsen. The primary endpoint — sustained HBsAg loss at 24 weeks post-treatment — was met in both studies. In the subgroup of patients with baseline HBsAg levels ≤1000 IU/mL, the functional cure rate was approximately 30 percent, a figure that represents a transformative advance over the ~1 percent annual cure rate associated with existing nucleos(t)ide analogue therapy (Entecavir and Tenofovir).

The mechanism of action distinguishes Bepirovirsen from all existing therapies. As an antisense oligonucleotide, it directly targets and degrades all HBV messenger RNAs, including those encoding HBsAg and the HBV polymerase. This dual action reduces viral protein production and, critically, reactivates the immune system’s ability to recognize and clear infected hepatocytes. The drug also engages Toll-like receptor 8 (TLR8), stimulating innate immune responses that further contribute to viral clearance.

The safety profile was generally manageable. The most common adverse events were injection-site reactions and transient liver enzyme elevations (ALT flares), which in many patients reflected immune-mediated viral clearance rather than hepatotoxicity. Serious adverse events were infrequent, and GSK reported no new safety signals compared to earlier Phase 2 data.

2. FDA Priority Review and Breakthrough Therapy Designation

The FDA’s decision to grant both Breakthrough Therapy Designation and Priority Review signals the agency’s recognition that Bepirovirsen addresses a serious and unmet medical need. Breakthrough Therapy Designation provides for intensive FDA guidance on trial design and a rolling review of the application. Priority Review shortens the review timeline from the standard 10 months to approximately 8 months. The PDUFA target action date of October 26, 2026 means a regulatory decision is expected before year-end.

Industry analysts note that Breakthrough Therapy Designation is reserved for therapies that demonstrate “substantial improvement” over existing treatments — a threshold that the 30 percent functional cure rate in HBsAg-low patients comfortably exceeds. The designation also facilitates accelerated pathways for supplemental indications, which could allow GSK to expand the drug’s label to broader patient populations in subsequent years.

3. Chia Tai Tianqing Secures China Commercialization Rights

The May 11, 2026, partnership between GSK and Sino Biopharmaceutical’s Chia Tai Tianqing subsidiary for exclusive China rights represents a strategic alignment of global innovation with domestic commercial reach. Chia Tai Tianqing is China’s leading liver disease pharmaceutical company, with an established sales force covering hospitals and specialist hepatology centers across all provinces. The financial terms were not fully disclosed, but the deal is understood to include upfront payments, development milestones, and tiered royalties on net sales.

Why It Matters

Hepatitis B is one of the world’s most significant and underappreciated public health challenges. Approximately 300 million people globally are living with chronic HBV infection, and the disease is responsible for an estimated 820,000 deaths annually — primarily from cirrhosis and hepatocellular carcinoma (liver cancer). Despite the availability of an effective preventive vaccine since 1982, chronic infection rates remain stubbornly high, particularly in sub-Saharan Africa, East Asia, and Southeast Asia.

In the United States, approximately 640,000 adults live with chronic hepatitis B, many of them undiagnosed. The Centers for Disease Control and Prevention estimates that only about 25 percent of infected individuals are aware of their status. Current treatment options — primarily the nucleos(t)ide analogues Entecavir and Tenofovir — are highly effective at suppressing viral replication but rarely achieve a functional cure. Patients must remain on therapy indefinitely, with regular monitoring for drug resistance and liver disease progression.

Bepirovirsen’s potential to achieve a functional cure in 30 percent of eligible patients represents a paradigm shift. A functional cure eliminates the need for lifelong medication, reduces the risk of liver cancer, and resolves the social stigma and psychological burden associated with chronic viral infection. For the global hepatology community, which has pursued a functional cure for hepatitis B with limited success for decades, Bepirovirsen validates the ASO therapeutic approach and opens a new chapter in antiviral medicine.

The implications extend beyond hepatitis B itself. The successful application of antisense oligonucleotide technology to achieve a functional cure in a chronic viral infection may accelerate ASO development programs targeting other persistent viruses, including HIV and hepatitis D. The biotech industry is watching closely.

China Industry Impact

China’s hepatitis B burden is the largest of any single country. Approximately 70 million people are chronic HBV carriers, representing roughly one-quarter of the global total. Despite China’s universal infant hepatitis B vaccination program — which has dramatically reduced new infections among children — the legacy burden among adults born before the vaccine era remains enormous. Liver cancer is the fourth most common cancer in China and the second leading cause of cancer death, with chronic HBV infection accounting for the majority of cases.

The Chia Tai Tianqing partnership is a critical enabler for Bepirovirsen’s China market penetration. Chia Tai Tianqing, a subsidiary of Hong Kong-listed Sino Biopharmaceutical (HK: 01177, market capitalization approximately HK$80 billion), has built the dominant commercial platform for liver disease therapeutics in China. The company’s existing hepatitis B product portfolio, including generic Tenofovir and Entecavir formulations, generates billions of RMB in annual revenue and provides a ready-made distribution network covering over 3,000 hospitals with hepatology departments.

From GSK’s perspective, the China market represents the single largest commercial opportunity for Bepirovirsen globally. Even a modest penetration rate — treating 1 to 2 percent of China’s 70 million chronic carriers over the first five years — could generate annual revenues in the billions of dollars. The partnership structure, with Chia Tai Tianqing handling local commercialization and GSK supplying the drug substance, mirrors successful models employed by other multinational pharmaceutical companies in China.

The deal also reflects a broader trend in China’s pharmaceutical industry: the growing willingness of domestic companies to license or co-develop innovative biologic therapies with multinational partners, rather than pursuing purely generic or me-too strategies. Sino Biopharmaceutical’s investment in the Bepirovirsen partnership signals confidence that China’s healthcare system is prepared to pay premium prices for transformative therapies — a significant shift from the price-driven procurement environment that has dominated China’s pharmaceutical market since the Volume-Based Procurement (VBP) policy was introduced in 2018.

For China’s Healthy China 2030 initiative, which targets a reduction in chronic disease burden and improved cancer outcomes, Bepirovirsen arrives at an opportune moment. The National Health Commission has identified hepatitis B elimination as a public health priority, and a therapy that offers functional cure to a meaningful patient subset would significantly advance that goal.

Supply Chain Implications

Bepirovirsen’s manufacturing and distribution present several supply chain considerations that differentiate it from conventional small-molecule drugs.

Antisense Oligonucleotide (ASO) Manufacturing. Unlike traditional pharmaceuticals, ASOs are synthetic nucleic acid molecules that require specialized manufacturing capabilities. The production process involves solid-phase oligonucleotide synthesis, followed by purification steps including ion-exchange chromatography and tangential flow filtration. GSK and Ionis have invested heavily in dedicated ASO manufacturing capacity, but scaling to global demand — potentially hundreds of millions of doses annually if the drug achieves broad adoption — will require significant expansion of production facilities. Ionis’s existing manufacturing site in Carlsbad, California, and GSK’s biologics facilities in the UK and Belgium are expected to serve as primary production hubs.

Cold Chain and Storage Requirements. ASO therapeutics typically require refrigerated storage (2-8°C) and stable cold chain management from manufacturing through to point-of-care administration. This presents logistical challenges in tropical and subtropical regions with high hepatitis B prevalence — particularly sub-Saharan Africa and rural Southeast Asia — where cold chain infrastructure may be limited. GSK’s experience with vaccine cold chain logistics (the company is the world’s largest vaccine manufacturer by revenue) provides a significant advantage in navigating these distribution challenges.

Diagnostic Testing Demand. Bepirovirsen’s efficacy profile — with the 30 percent functional cure rate concentrated in patients with baseline HBsAg ≤1000 IU/mL — creates a strong incentive for pre-treatment HBsAg quantification testing. This will drive demand for HBsAg quantitative assays (qHBsAg) across hepatitis B treatment centers globally. Diagnostic companies including Roche Diagnostics, Abbott Laboratories, and Chinese players like Sansure Biotech (688289.SH) and Da An Gene (002030.SZ) stand to benefit from increased testing volumes.

Monitoring Infrastructure. Patients receiving Bepirovirsen will require regular monitoring of HBsAg levels, HBV DNA, liver function tests (ALT/AST), and HBV serology markers during and after treatment. This monitoring requirement generates sustained demand for diagnostic reagents and laboratory services — a positive tailwind for China’s in-vitro diagnostics (IVD) sector, which has been seeking growth drivers beyond the COVID-19 testing windfall.

CII Analysis

Our Take: Bepirovirsen represents the most significant advance in hepatitis B therapeutics in three decades. The Phase 3 data validate the antisense oligonucleotide mechanism for functional cure of a chronic viral infection — a breakthrough that extends well beyond hepatitis B itself. The 30 percent functional cure rate in patients with HBsAg ≤1000 IU/mL is transformative, but the critical question for commercial success is the addressable patient population: approximately 40-50 percent of chronic HBV carriers fall into this low-HBsAg category, meaning the drug’s peak commercial potential is substantial but not universal. The China partnership with Chia Tai Tianqing is strategically shrewd — it accesses the world’s largest patient pool through a proven commercial platform while insulating GSK from the complexities of China’s hospital procurement system. If approved by the October 2026 PDUFA date, Bepirovirsen could achieve peak global sales of $5-8 billion, with China accounting for a substantial share. The risks are real: long-term safety in larger populations, durability of cure beyond 24 weeks, and the potential for resistance or relapse. But the weight of evidence — Breakthrough Therapy Designation, Priority Review, two positive Phase 3 trials — tilts the probability heavily toward approval. This is a watershed moment for hepatology and for the broader ASO platform.
